Multiple county service is optional but not required.
Counties include but are not limited to:
Mahoning and Columbiana.
SummaryProvides physician-ordered physical therapy services as a Physical Therapy Assistant, which include documentation, assessment, planning, developing, and implementing an individualized patient therapy plan.
This Physical Therapy Assistant position carries out the physical therapy plan with the interdisciplinary team care plan.
With INFINITY, You will have quality time with ALL of your caseload to promote their health and facilitate their independence!!!!!Essential Responsibilities/Accountabilities of a Physical Therapy Assistant Provides physician-ordered physical therapy services as a Physical Therapy Assistant.
Observe, record, and report patient short/long term goals in terms of functional outcomes as a Physical Therapy Assistant.
Reflect progress or lack of goal achievement in all charting.
Confers with physician and/or interdisciplinary team regarding patient response to therapy as a Physical Therapy Assistant.
Monitors changes needed to increase patient's independence as a Physical Therapy Assistant.
Recommends equipment and assistive devices (ramps, bathroom aides, adaptive wheelchair), as appropriate.
Instructs the patient and caregivers in therapeutic exercise, which may be used to improve the patient's functional status as a Physical Therapy Assistant.
Provides instruction in the use of various assistive devices (orthotic appliances, crutches, walkers, cane, etc.
) as a Physical Therapy Assistant.
Treats patient's physical deficits using clinically accepted therapeutic exercises.
Utilizing techniques in the plan of care as a Physical Therapy Assistant.
Instruct patient and caregivers in safe transfers (bed, chair, commode, etc.
) using appropriate body mechanics and equipment (sliding board, Hoyer lift, trapeze, bath bench, etc.
).
Attends scheduled interdisciplinary staff meetings and actively participates in treatment planning PRN as a Physical Therapy Assistant.
Consistently demonstrates high standards of integrity by compliance with the Code of Ethics and State Laws.
Maintains high regard for member privacy in accordance with HIPPA privacy policies and procedures.
Regular and reliable attendance is expected and required.
Performs other functions as assigned.
Minimum Qualifications Degree from an accredited Physical Therapy school with current Ohio licensure as a Physical Therapy Assistant.
Ability to travel required.
Possession of a valid Ohio motor vehicle operator's license.
Pre-employment physical examination and an annual health assessment as required by Ohio State Health Department.
Current CPR certificationPhysical RequirementsAbility to travel as necessary in order to offer support for on-site visits, training, and/or off-site meetings.
